To better serve our Veteran patients, Delaware Hospice participates in the We Honor Veterans program as a Level 4 Community Partner. Our participation helps ensure that the unique needs of our Veteran patients and families are being met through a Veteran-centric approach to care. We Honor Veterans is a partnership between the Department of Veterans Affairs (VA) and the National Hospice and Palliative Care Organization (NHPCO). This partnership provides the tools and resources for end-of-life care organizations to meet the needs of their Veteran patients.We Honor Veterans provides educational tools and resources to organizations in advancing these goals: • Promote Veteran-centric educational activities• Increase organizational capacity to serve Veterans• Support development of strategic partnerships• Increase access and improve quality800-838-9800 | delawarehospice.orgDelaware Hospice is accredited by the Joint Commission and provides care throughout Delaware and southern Chester and Delaware Counties in Pennsylvania. Veteran-Centric care addresses the individual needs of those who have served by adopting an approach to care that is centered in a Veteran’s unique history, experience, and needs.94% of Veteran end-of-life care is accessed in the civilian sector which is why having specially trained staff is critical when caring for the needs and qualities of this population.
